I understand there is a $25 per dog cancellation fee and a $15 per cat cancellation fee, which Safe Dog Express will happily credit toward my next transport: 
I understand that all pets must have a collar with good ID and a health certificate issued within 10 days before our transport:  
I have read all the 'Doggy Requirements' listed on the Safe Dog Express website, and my pets will meet these requirements before boarding the transport:
I hereby allow a veterinarian to care for my dog in the event of an emergency enroute, and I understand I am responsible for that cost.
